Head of Engineering
Software Engineering · Full-time
Boston, MA, USA
Why We Exist
Within a few years, AI will handle a large share of the work that happens on a medical practice's phone lines. Most of healthcare is not ready for that, and most healthcare AI is not built for it. The phone call is still where care often begins, and it still runs on voicemail, callbacks, and notes typed from memory.
Lucea is building the layer that sits inside those conversations. Today we capture practice phone calls and turn them into accurate documentation that staff can trust. From there, we bring licensed clinical protocols into the call, give practice leaders visibility into their operations, and automate more of the work as quality is proven, always with clinicians in control of clinical decisions. We started with pediatric nurse triage, one of the hardest-to-staff jobs in nursing, and we are expanding across practice types and roles.
The early results are strong. A small team has supported more than 25,000 live triage calls and converted every design partner into a paying customer. A triage nurse four days into using Lucea told us: "Don't take this away from me." We are hiring the people who will take it much further.
Why This Role Matters
- We are looking for a rare kind of engineering leader: someone who spends their own time with the newest models and agentic frameworks because they want to know what's possible next, and who can also run a production healthcare platform that clinicians rely on during live patient calls.
- Lucea's next chapter needs both. We have a product that practices can buy, connect to their phone systems, and use every day, and we're laying the foundation for what comes next: clinical assistance, practice analytics, and voice automation that takes on more of the work as quality is proven. The architecture, team, and engineering culture you build now will decide how fast we can move as model capabilities compound.
- You'll set Lucea's technical direction and build an engineering team that works the way the best AI-native teams do: small, fast, and multiplied by agents and tooling you help design. You'll make the architectural bets that let our product get better every time the models do, and you'll know when a new capability is ready for real practices.
- You'll work directly with our CEO on product and strategy, hire the engineers who will help define Lucea, and stay close enough to customers to feel their pain firsthand. If you care more about where AI is going than where it is today, and you want to prove it in one of the hardest and most important settings there is, we want to talk with you. We're based in Boston, but we want to work with the best in the world.
What You'll Own
AI Systems and Engineering Leverage
- Own how Lucea uses models: which ones, where, and at what cost. Keep the product improving as models do, and use less expensive models where they meet the quality bar.
- Build the evals and quality loops that show, with evidence, whether documentation is accurate, complete, and consistent, and whether a new capability is ready for real practices.
- Empower engineers and non-engineers to build and ship with the best models and development tools. Keep improving how the team works as those tools advance, and own the quality and security of what reaches customers.
Systems and Delivery
- Own R&D delivery end to end, from ambiguous customer needs to shipped, reliable software on a growing multi-tenant healthcare platform.
- Make phone system integration standardized and repeatable, so each new practice goes live faster than the last.
- Hold a very high bar for correctness and data integrity across integrations with EHRs, telephony, and third-party APIs.
- Understand the full cost of delivering the product, from telephony and transcription to model usage, and design for sustainable economics.
Reliability and Security
- Raise the bar on reliability: observability, alerting, incident response, and the deployment pipeline.
- Build security and compliance in by default (HIPAA, encryption, auditability), and set the standards the team follows.
Technical Leadership
- Set technical direction for the team and build alignment around it.
- Hire engineers who raise the bar, and build a culture that's ambitious about AI and rigorous about quality.
- Level up how the team builds through code review, design discussion, and pairing.
- Partner with clinical and product stakeholders to turn requirements into sound technical plans.
Customer Empathy and Product Excellence
- Know how nurses, providers, and practice staff rely on Lucea day to day, find the gaps, and get the fixes prioritized and shipped.
- Obsess over the details of the experience: present information more clearly, remove a click, make the next action obvious.
- Be the voice of the customer to the engineering team, so no one forgets who we're building for.
What Good Looks Like
- You bring energy and urgency to the work, and the team does something hard every day.
- Each new generation of models makes Lucea better, and adopting them is routine instead of a rebuild.
- We can show with evidence how good our notes are, and new capabilities reach customers once they've earned it.
- A small team ships far more than its size suggests, because AI is built into how we work.
- New practices go live on a standard, repeatable integration, and the platform scales across more practices and enterprise customers without reliability or security regressions.
- The cost to deliver the product comes down as usage grows.
- Our engineers get better because of your reviews, your standards, and the patterns you establish, and the people you hire raise the bar further.
- Our CEO and the team trust you with the technical calls, freeing others to focus elsewhere.
What We're Looking For
Must Have
- 7+ years building and owning production systems, including time hiring and leading engineering teams.
- You build with LLMs, not just around them. You've shipped LLM-powered features to production and handled what comes after launch: evals, quality drift, latency, and cost.
- You live at the frontier. You try new models and agentic tools as they come out, you have a point of view on where they're heading, and you can tell hype from a real step change.
- AI is how you build. You use AI coding tools and agents daily, and you've changed how a team works because of them: workflows, tooling, code review, and who does what.
- You've made architecture decisions that held up under real-world conditions, and you design systems that get better as models improve instead of needing a rebuild with each release.
- Exceptional software engineering fundamentals and experience designing large-scale systems. You have a strong intuition for how AI changes who does what on a team and how software gets built.
- You work well with autonomy in a fast-moving, ambiguous environment, and you bring others along with you.
- You know how to join an existing team and codebase, earn credibility with the engineers who built it, and raise the bar through influence and example as well as process.
Strong Signals
- Experience in healthcare, fintech, or other regulated industries, and comfort building for compliance (HIPAA, encryption, auditability).
- Experience with real-time voice systems: telephony, streaming audio, speech-to-text, or WebSockets.
- Experience building integrations with EHRs or other complex third-party systems, and making them repeatable.
Who This Role Is Not For
- You want to manage from a distance and leave the architecture, development process, and quality of what reaches customers to someone else.
- You measure success by how many people report to you.
- You want a fully researched PRD for every change instead of taking a problem and running with it.
- You see AI as a feature for the roadmap, or you're waiting for the tools and best practices to settle before you commit to them.
- You're excited by the newest model but not by the work of making it reliable for a clinician on a live call.
- You want to work on the interesting technical problems and leave the customer's pain to someone else.
Compensation
- Base salary: Competitive for a seed-stage health tech company.
- Equity: early-stage grant. You'll join early and build the engineering organization from here.
- Benefits: Medical, dental, vision, commuter, fitness, 401(k), HSA, unlimited PTO.
